Study in Journal of Respiratory Therapy shows superiority of D R Burton vPEP and confirms expiratory flow bias as the critical mechanism of action for respiratory airway clearance devices

–Results show importance of instructing patients to perform slow, prolonged inspiration during oscillating PEP therapy for secretion clearance–

The study utilized a human lung simulator to analyze the vPEP and two competitors against three measures of OPEP effectiveness: flow amplitude, expiratory pressure, and the ratio of peak expiratory flow to peak inspiratory flow. The study revealed the superiority of the vPEP against competitors in all measures, and confirms a previous study which identified expiratory flow bias as a key driver in moving secretions up the airway. Expiratory flow bias occurs in breathing when a patient’s peak expiratory flowrate exceeds their peak inspiratory flowrate.

“D R Burton is changing the paradigms by which respiratory professionals evaluate OPEP therapy,” says Dennis Cook, BSRT, RRT, and CEO of D R Burton. “This landmark study shows expiratory flow bias as a key driver of secretion mobilization, whereas the industry had previously emphasized the importance of pressure which hasn’t been proven for airway clearance,” says Cook.

Additionally the study revealed the importance for patients to perform a slow, prolonged inspiration during oscillating PEP therapy. A slow inspiration combined with a breath hold and forced exhalation will allow for the greatest possible expiratory flow bias and secretion clearance. Cook says, “The vPEP not only outperforms its competitors across the three measures in this study, but it’s also the first OPEP device with instructions for use that are aligned with this study.”

J-Wand

Along with a slow coached inhalation, the iPEP provides superior Expiratory Flow Bias, a key driver of secretion clearance outlined in a recently published study.

The iPEP system includes a palm-sized PocketPEP®, a compact OPEP device for active patients for use at home to help prevent pulmonary-related readmissions. Cook explains, “There is a need to prevent pneumonia in high risk patients, such as postoperative and patients with increased mucous production. The J-Wand was developed by Certified Registered Nurse Anesthetist, Jay Annis, who is also D R Burton’s Vice President of Anesthesia. This innovative design easily and reliably facilitates placement of an endotracheal tube (ETT) during difficult intubations.

The J-Wand stylet employs an anatomically inspired curved rigid stainless-steel stylet with flexible introducer tip. This combination speeds time to intubation, while also reducing risk of airway trauma, and improving patient safety. Mr. Annis has stated, “The J-Wand’s flexible introducer tip helps to guide the ETT towards difficult anterior airways more effectively than current rigid stylets and standard flexible stylets. It also gives practitioners the option of providing each patient direct apneic oxygenation during the intubation process.”

J-Wand

The J-Wand supports direct laryngoscopy or video laryngoscopy, and also features an apneic oxygenation port. Using apneic oxygenation techniques during difficult intubations can safely prolong the duration of apnea without desaturation. The J-Wand is packaged sterile, is disposable, and is a single patient use product.
